请问收到最下面这封邮件，该怎么填写
1 之前就是用公司注册的，是下面的填写吗， Name of organization 这个填写公司的英文名字就可以了吗
2 Permanent Address 这个之前写的是老家地址，不是营业执照地址，需要改成营业执照地址吗，改的话会不会触发二审
